There are many, but some include attendance, immunization, incident/accident/injury, emergency contacts, diapering and feeding log for infants, observation records, student work, etc.

each child has a file with medical information, immunization records, emergency contact information, food allergies or watch list. injury or sickness reports will also be in this file too

Name, address, DOB, immunization records, phone number, parents work and home address and telephone. Emergency contacts. People who have permission to pick child up.

I must maintain up-to-date medical and health records including immunization histories, injury reports, child information and allergies, emergency contacts, and medication authorizations.

i am required to keep attendance records, meal records, injury and illness records, when we go to an unlicensed space i am required to fill out a field trip form. I am also responsible for keeping each child's file up to date and accurate.
