Observing a child and assessing them based on developmental milestones in their physical, emotional, mental, and intellectual health. Compiling specific observations in behavior and skill, and how they translate into/support your assessment of the child's development will help the parents better understand their child's development. Keeping a portfolio of the child's work is also helpful, so parents can see for themselves the improvements a child has made throughout the month/term/year. A professional communication of information should never seem judgmental, but should point out concerns and ways to help remedy them after displaying positive progress.
