Each child's record must include: - complete enrollment application - name, birth date, dates of enrollment/termination, other identifying info - name, address, home and business telephone number of parent and other emergency contacts - health history which includes date of last physical, allergies and symptoms + treatmenet, health/development concerns, medical conditions, medications, name, address, and ph# of health care provider, and dentis - individual plan of care for chronic health conditions/life threatening medical conditions - written consent from parent for emergency medical care - how to contact parents or other family/designated emergency contacts in an emergency - instructions from parents or health care provider related to medications, specific food/feeding, allergies, treatments, special equipment, or health care needs - written records of illness or injury that occurs during childcare hours and treatment provided - written records of medications given while in child care - authorizations: 1. name, address, and ph# of persons authorized to remove child from the center 2. parental consent for transportation to/from school 3. parental consent for transportation provided by center to/from field drops - immunization status up to a year after the child leaves care
