Some of the things that make an indoor environment safe for children is keeping things sanitized, picking up toys and repairing and flooring or carpeting issues to prevent trips, all toys are within reach to prevent climbing, cabinets and doors are child-proofed, constant adult supervision etc.
Some of the things that keep and outdoor environment safe are constant adult supervision, all equipment is maintained and meets safety regulations, water tables have no more than 1/2 in of water, children use sunscreen (with proper permission slip signed by parents) when going outdoors, etc.

A safe indoor environment includes keeping everything as clean and sanitized as possible, no toys left out or carpets lifting up to insure that no one trips and falls. Allowing all necessary items to be in reach for children. A safe outdoor environment includes supervision at all times,and meets all safety standards.

A safe outdoor environment includes a place where the care providers can supervise at all time and that the equipment meets all of the standards. A safe indoor environment includes keeping everything clean, and sanitized. All toys are put away so they children and yourself do not trip and get hurt.

What makes a safe indoor environment for young children is having children size furniture and arranging furniture so children are not likely to trip over or run into it. Furthermore, storing toys and books within easy reach of children, removing broken equipment, storage substances or materials that can cause poison or chocking hazard properly and keeping away and inaccessible from children’s reach. Additionally, keeping the environment, toys and materials properly clean, store children’s individual personal items properly, provide proper temperature/ventilation and so on. What makes an outdoor environment safe for young children is having the outdoor play area properly fenced, and provide materials and equipment that is safe for the children and meets all of the appropriate standards. Furthermore, an outdoor area also requires careful supervision from a childcare provider while children are playing there.

What makes an indoor environment safe is not having a room to over crowded with equipment and toys to the point where there's not enough safe room to walk around. Making sure you have adequate enough storage space to put away toys, games, and indoor play equipment. Making sure that things that should be locked are locked at all times such as cabinets that may contain hazard materials and cleaning materials.
What makes an outdoor environment safe is that the play is well secured and you have adequate amount of supervision. All playground equipment is kept up and if anything is broken it should be sealed off so kids can't get to it.

For indoor safety, the proper storage of chemicals for cleaning, secure storage for medicine, separate storage for food, mounted to wall shelves, secure hanging objects, lighting safety, toy safe for choking hazards, storage of school supplies, sanitizing the toys and founiture in the proper procedures base on WAC and more.
For outdoor, weekly play equipment hazards, weekly outside check up for potential hazards for everyone, secure gates, clean toys, falling zone safety and more.

Safe indoor environment: Clean and age appropriate toys. Hazardous chemicals up and out of reach from the children.
Safe outdoor environment: Age appropriate structures, locked and gated properly, correct child top teacher ratios.

What makes an environment safe both indoors and outdoors is that all items are in good repair, cleaned regularly and age appropriate to avoid injuries as much as possible.
